These 8.5 x 11 Laser Path
Req label sheets are designed to reduce processing errors while integrating
with Modernizing Medicine EMA software, and other customizable templates. Each
page has 6 integrated detachable labels for specimen biopsy samples and other
lab or filing needs. Once the labels are detached the top portion can be kept
in a patient file or submitted to the lab.
Specifications:
Sheet size is 8.5 x 11 white
laser compatible stock.
Six 2.5" X 7/8" Integrated labels at the bottom
Label block is 1/2" from the left and right - 1" from the bottom
Works with Mod Med EMA software and custom templates including Microsoft word.
Available in packs of 250 or by the case of 1750 sheets
Custom Imprinting
Sheets can be custom pre-printed in one standard
color ink with your company information on them so labs or other offices can
easily identify who is submitting the forms.
Imprinting cost is $50 for the first 1000 and $35 for each additional
