Each patient prints his/her name, appt/arrival time and “Name of
Healthcare Professional” on a numbered line. Once the label has been
removed, the information is protected behind part one, available only
for office use.
As patient travel becomes more vital to illness diagnosis, TWI offers a timely update to our Patient Sign-in Label Sheets. Without forgoing any of the original questions, TWI has added an "Out of the Country Recently" check box.
