These 8.5 x 11 Laser Path Req label sheets are designed to reduce processing
errors while integrating with NextGen EMR Pathology software, and other
customizable templates. Each page has 8 integrated detachable labels for
specimen biopsy samples and other lab or filing needs. Once the labels are
detached the top portion can be kept in a patient file or submitted to the
lab.

Custom Imprinting
Sheets can be custom pre-printed in one standard color ink with your company
information on them so labs or other offices can easily identify who is
submitting the forms. Imprinting cost is $50 for the first 1000 and $35
for each additional 1000.
